• Air Ambulance or Medical Escort:  This section should be completed if the clinician has every worked for a medical escort or air ambulance company in the past two (2) years.  Disclosing this information will not eliminate you from any travel assignment offered by Sky Nurses unless the clinicians holds a non-compete with the company with whom he/she has a relationship with.  However, omitting this information from the application will be cause for termination of employment or contract.
  
• Education:  the applicant must complete the education reference to his/her degree in medicine.
• Employment History:  the applicant should list the last place of employment.  A resume should be submitted prior to completing the application and should contain an employment history for the past 7 years.
• Professional References:  the applicant must provide two professional references.  This should be someone that has worked with you or someone that you have worked under.
